Intravenous immunoglobulin (IVIg) is a treatment option for refractory myasthenia gravis (MG). We have evaluated a protocol of IVIg therapy for patients with severe MG, and we report the experience of the Buenos Aires Myasthenia Gravis Center. Between 2001 and 2009, 30 patients (14 men and 16 women) were treated with IVIg. Thirteen patients were seronegative, and 17 had antibodies against the acetylcholine receptor. Eleven patients required mechanical ventilation and 21 had dysphagia. After 10 days of IVIg therapy, 14 patients achieved complete remission, while 5 had improvement. At the end of the follow-up, 4 of the 17 patients with anti-acetylcholine receptor antibodies had complete remission, while 8 improved. Two seronegative patients improved after 20 days of IVIg. Adverse effects were pruritus, transient fever and transient nausea and vomiting. Two patients with MG received IVIg therapy without improvement, and one seronegative patient was able to achieve remission after a second course of IVIg.
